What do I need from the library in order to complete my course additions forms?

1. Print a copy of the Library Course Addition Survey Form (MS Word) and fill it out.

2. Determine which librarian is responsible for your subject area, by looking at this list of subject areas.

3. Send your completed Library Course Addition Survey Form to the appropriate librarian, along with a copy of your other course addition forms (filled out), as well as the course outline.  Please get these documents to the librarian ahead of time - before you schedule an appointment.

3. Schedule an appointment with the appropriate specialist librarian.
